How to Access Your Information

To log in and access your account information you will need 3 pieces of information as shown in Figure A Below:

• Last Name
• User ID
• Password

In just four easy steps you will be ready to access your account.
Step 1. Call our office and ask for your unique User ID.
Step 2. Click on the "Log In" link at the top of this page
Step 3. When the "CLIENT LOGIN" page has loaded click on the New Users link as seen in Figure B below
Step 4. Now enter your first name, last name and User ID as seen on Figure C below. You will now be given your private password.

Once you receive your Password you are ready to access your appointment and account information. If you have any questions please contact our office at (831) 475-5500.
